- Valid Certificate of Incorporation
- Form C07 showing names and particulars of director(s) and radiographer(s)
- Radiographer In Charge Passport Photograph
- Radiographer In Charge Current Practice Licence
- Payment of Radiography Practice Permit renewal fee of (N50,000.00) to be paid to RRBN TSA Account:

NOTE: Radiography Practice Permit is valid for One (1) Year
Application Process Flow
- STAGE 1: Hospital Profile Creation (for first time users of the platform). Already registered users should login to their dashboard to start practice permit renewal
- STAGE 2: Submission of basic hospital/centre details, certificate of incorporation, form C07, Radiographer In Charge passport and Practice Licence and evidence of payment of cost of available modalities.
- STAGE 3: Verification of submitted documents and modalities fees payment.
- STAGE 4: Final Approval by RRBN Registrar/CEO.
- STAGE 5: Radiography Practice Permit Issuance
